Sponsored Residential provides 24-hour support in an approved provider home. The individual becomes part of a welcoming household while receiving individualized assistance based on their needs and service plan.

Supported Living
Supported Living assists individuals living in a home or apartment setting with 24-hour staff availability. Support is individualized and designed to strengthen everyday skills, increase confidence and promote greater independence.

For Support Coordinators and Refferal Partners
Bridgeway welcomes inquiries from support coordinators, case managers, discharge planners, healthcare professionals and other members of an individual’s support team. Placement is based on eligibility, assessed needs, compatibility, available capacity and successful completion of Bridgeway’s referral and admission process.
